Plan Disclosures & Legal Terms

The Direct Dental Savings Plan (the "Plan") is an in-house discount dental plan and IS NOT INSURANCE. The Plan is not a qualified health plan under the Affordable Care Act (ACA). Plan members pay a periodic membership fee in exchange for access to reduced fees on certain identified dental services rendered exclusively by Happy Teeth & Affordable Implants in accordance with the plan fee schedule.

Plan members do not receive insurance reimbursement and are obligated to pay dental providers directly for all services rendered at the time of treatment. Plan details, retail fees, and member savings apply strictly to clinical procedures performed at our practice and do not apply to external laboratory fees, retail products, or specialist referrals. Membership fees are non-refundable once clinical services have been rendered.

The Plan is administered directly by Happy Teeth & Affordable Implants and is not affiliated with, approved by, or endorsed by the Colorado Division of Insurance or any state insurance department.
